Mosaic

Mosaic is a whole-person healthcare organization reaching across 13 states. Mosaic and its subsidiaries provide support to nearly 4,5000 people. Services empower people with disabilities, mental and behavioral health needs and autism, as well as aging adults to live their best life.

Currently Mosaic in Omaha offers the following services:

• Mosaic at Home partners with families throughout Omaha and the surrounding areas, known as Shared Living Providers, who open their homes to individuals we serve and provide 24/7 support and individualized care.
• Day Services Program, a place for our Mosaic at Home families to take their individuals during the day. At Day Services, people receive a variety of therapeutic services to foster life skills for independence that promotes their physical and social health. They also go out into the community for outings and community partnerships to foster community inclusion.
• Intermediate Care Facilities, three homes throughout the Omaha area that offer a type of long-term care that provides residents with around-the-clock, supportive nursing care.
